Every patient is unique, and we are committed to providing you with quality periodontal and implant therapy based on your individual needs. Above all else, we want to make your visit to our office a pleasant experience.

What to Bring on Your First Visit

Please arrive 15 minutes early for your appointment so you have adequate time to complete our initial forms. If you have access to a computer, you can follow the link below to download patient forms, and complete them prior to your visit. If you have dental insurance, please bring your dental insurance information to your first visit.

You may also want to write down any questions or concerns you have regarding your dental health and bring this list with you. This will help to ensure all your concerns are addressed.

What to Expect on Your First Visit

Your first visit will include the following: 

  • Review of your complete dental and medical history form
  • Review of your dental x-rays and/or taking new x-rays as needed
  • Examination of hard and soft oral tissues

After a thorough exam, we will discuss our findings and answer any questions you may have about treatment. Our goal is to help you come to an informed decision regarding care.

Antibiotic Premedication

Patients with certain medical conditions (e.g. valvular heart conditions and recently placed artificial joints) may be asked to premedicate with antibiotics prior to examination, cleanings, or surgical therapy. We do this with your safety in mind. Please inform us if you have a condition that might need antibiotic premedication.
